  • Overview

    The hospice Volunteer/Bereavement Coordinator plans, organizes, and directs the recruitment, screening, training, and utilization of volunteers for the company’s Hospice Program as well as coordinating the bereavement process and Memorial services.


    • Screens all patients within 7 days of admission and identifies whether patient/family desires need for volunteer. Utilizes the 3 Key expectations the patient identifies in order to provide excellent care volunteer care.
    • Assigns volunteers to meet program needs and volunteer interests and skills.   Accompanies the volunteer on the first time visit to introduce patient and volunteer.
    • Arranges volunteer meetings for support, on-going training, problem solving and social functions.
    • Recruits, screens, and selects volunteers for the Hospice Program. 
    • Develops, coordinates, and participates in a training program for volunteers and staff.
    • Assesses and monitors the Volunteer Program’s needs and develops appropriate volunteer activities.
    • Organizes monthly Caregiver Support Groups at specified nursing centers with monthly topics related to Caregiver Needs.  Promotes Support Groups utilizing the Chaplain, Social Worker, Volunteers as needed.  Assists with community / professional awareness activities as they relate to the hospice program.
    • Assists with the definition, development, and evaluation of the volunteer’s role with the plan of care.
    • Serves as a member of the Interdisciplinary Team (IDT) and attends IDT team meetings.  Updates the interdisciplinary plan of care at least every two weeks. 
    • Participates in Performance Improvement activities.
    • Plans, advertises and implements the annual memorial service.
    • Oversees completion of documentation on each individualized bereavement care plan.  Reviews existing bereavement care plan with the IDT to revise and update.
    • Participates in the bereavement process by coordinating bereavement telephone contacts with families.
    • Willing and able to accurately utilize any automated device required to document in the electronic medical record.


    • Bachelor’s Degree in a behavioral or social science area preferred. 
    • One (1) year experience in a related field and volunteer activity preferred. 
    • One year supervisory experience preferred.
